Resource Coordination

To support individual and family choice by working together
to maximize opportunities, resources and community involvement.
Resource Coordination is a service of Developmental Disabilities Resource Center that assists individuals and families with eligibility determination and assessment, individual plan development, information and referral, assistance in applying for Medicaid Waivers and monitoring of services and supports. Resource Coordination provides case management services to over 2500 children and adults both enrolled and waiting for services.

 

A Resource Coordinator is assigned to each person requesting service. Their role is to provide assistance as needed to individuals and families to identify desired outcomes and the resources needed for them. Resource Coordinators strive to maximize support options in the individual's natural community. Through information, education and advocacy, Resource Coordination partners with families and individuals to safeguard rights and assure due process. Choice of Resource Coordinator is welcomed and available upon request.

 

Resource Coordination Services is located in the DDRC Administrative Office at 8th and Quail in Lakewood, Colorado, on the fourth floor. Under its umbrella, Children and Family Services provides resource coordination to children and their families and administers the Family Support Services

Nursing home
